1. PARTICIPATE

Participate in 100% of the session(s) you attend.

2. EVALUATE

Complete the evaluation emailed to you Friday, November 6th.

The evaluation closes at 11:59PM CST Sunday, December 6th. No exceptions, no extensions.

3. RECEIVE

You will receive your certificate(s) via email by December 31st. The amount of CEs awarded will be based on the number of sessions you attended and evaluated.

The evaluation will be sent to the email address you used to register for the Annual Conference. Check your SPAM folder before emailing ashrmed@aha.org. Look for an email from SurveyMonkey as that is the platform used for the evaluation.

Evaluation must be completed by 11:59PM CST Sunday, December 6th. No exceptions, no extensions.

Attendees are responsible for the following:

• Tracking the sessions you attend
• Submitting CEs to the accrediting organizations listed on the back
• Completing the evaluation by 11:59PM CST Sunday, December 6th

American Society for Health Care Risk Management (ASHRM)

This program has been approved for 11.25 contact hours of continuing education credit toward fulfillment of the requirements for ASHRM designations of FASHRM (Fellow) and DFASHRM (Distinguished Fellow), and toward CPHRM renewal. (Type code: 1, Content codes: 1, 2, 3, 4, 5).

American Nurses Credentialing Center's Commission (ANCC)

American Society for Health Care Risk Management is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation. This continuing nursing education activity was approved for 11.25 contact hours.

You must specify the additional accrediting organization(s) from which you wish to receive CEs in the evaluation. You will receive an email with your certificate(s) for the specified additional accrediting organization(s) by December 31st.
